Selecting the right implant is an important part of your breast augmentation journey. At Colorado Plastic Surgery Center, board-certified Denver plastic surgeon Dr. Nick Slenkovich and board-eligible plastic surgeon Dr. Laura Roider are here to help guide you through that decision. Both combine advanced surgical training with a patient-centered partnership model that prioritizes education and transparency.

One of the first choices when selecting your implant is Silicone vs Saline

Both saline and modern silicone implants are FDA-approved and considered safe, but they differ in feel, appearance, incision requirements, and long-term monitoring.

In most cases, silicone implants offer a softer, more natural look and feel. They are especially beneficial for women with thinner breast tissue. Saline implants are filled with sterile saltwater after placement and may allow for a slightly smaller incision, yet they are more prone to visible rippling. Understanding these distinctions early helps clarify which option aligns best with your anatomy and goals. In this blog, we outline what truly matters before making your decision.

The Pros and Cons of Saline Implants

Pros:

  • Smaller incision: Because saline implants are inserted empty and filled once in position, the incision can be slightly shorter.
  • Immediate rupture detection: If a saline implant leaks, it deflates quickly, and the body harmlessly absorbs the saline solution.

Cons:

  • Firmer feel: Especially in patients with minimal tissue coverage, saline can feel less natural.
  • Increased rippling risk: Visible wrinkling is more common in thinner individuals and saline implants.
  • Noticeable deflation: If rupture occurs, volume loss is obvious, and surgical replacement is required to address the resulting asymmetry.

The Pros and Cons of Silicone Implants

Pros:

  • Natural look and feel: The cohesive gel provides a softer, more tissue-like consistency.
  • Lower rippling risk: Silicone is often preferred for women with thinner frames or limited native breast tissue to decrease the risk of visible rippling/wrinkling.
  • Shape stability: Even if the outer shell is compromised, the gel typically maintains its form. This means a rupture doesn’t require urgent intervention.

Cons:

  • Slightly larger incision: Silicone implants are pre-filled and require marginally more space for placement.
  • Silent rupture possibility: If rupture occurs, the breast may not visibly change, which is why routine imaging is advised.

Safety, Longevity, and Long-Term Monitoring

Both saline and silicone implants carry similar surgical risks, including capsular contracture, infection, changes in sensation, and rupture. Neither type should be considered a lifetime device, and some patients will require revision surgery at some point due to aging, lifestyle changes, or implant integrity.

The FDA recommends periodic imaging for silicone implants to evaluate shell integrity. This first screening is recommended 5-6 years after placement, then every 2 years thereafter.  Both Dr. Roider & Dr. Slenkovich emphasize s proactive monitoring and patient education, ensuring individuals feel informed and supported throughout the lifespan of their implants. With appropriate monitoring and surgical technique, many patients enjoy beautiful, stable results for well over a decade.

Choosing the Right Implant for Your Body

Implant selection is never one-size-fits-all. Tissue thickness, chest wall anatomy, lifestyle, and long-term expectations all influence the recommendation.

At Colorado Plastic Surgery Center, breast augmentation is approached as a collaborative process rooted in expertise and integrity. Dr. Slenkovich and Dr. Roider take the time to evaluate your anatomy, explain your options clearly, and recommend the safest and most appropriate option for you.
